Applying for OSHA Card Replacement How to Apply for Replacement Cards for Lost, Damaged or Worn Out OSHA Cards Over time, OSHA course completion cards may be lost, damaged, or worn out. The procedure for applying to OSHA for a replacement card should be done as soon as possible. The OSHA course completion card is important to replace because it is validation of OSHA training courses successfully completed. Steps to OSHA Card Replacement Contact your OSHA Outreach trainer. Be sure to apply within five years of your training course program completion. Contact OSHA online or by phone at the toll-free number for questions. If the card is damaged or worn out, the Outreach Trainer will advise on whether you need to destroy it or return it to OSHA Outreach. If you require replacement of your certificate of course completion, simply go to âMy Trainingâ page at www.oshacampus.com and click the certificate icon. If all course requirements are completed, you can view and print your certificate of completion for replacement. If your card is more than five years old, you will need to renew your course training in order to receive a certificate of completion and a new OSHA wallet card.
